<p>When a root canal filling develops a granuloma, cyst, or some other infected area at the end of the root, dentists will sometimes endeavor to save the tooth by performing an operation called apicoectomy. The area of infection seen on x-ray pictures is actually a hole in the bone of the jaw eaten away by bacteria and toxins. It contains pus, bacteria and infected tissue.</p>
<p>The apicoectomy surgery is done using a local anesthetic. An incision is made in the gum and the dentist invades the infected area and curettes away the diseased tissue.</p>
<p>In order to be able to remove all infected tissue surrounding a tooth&#8217;s root end, it is sometimes necessary to also remove a portion of the tooth&#8217;s root end (apex). This is done with a surgical dental burr or drill. The term apicoectomy was adopted because the end tip of the root is so often removed during this procedure.</p>
<p>Two or three stitches are used to close the wound. These areas experience some swelling for two or three days but generally heal with very little discomfort. Usually new bone immediately begins to grow and fill in the jaw at the end of the root, and after six to 12 months one can no longer distinguish the location of the infection sight. In other words, the area&#8217;s appearance is now normal.</p>
<p>At times when cysts or other large areas of infection are found, dentists will elect to do the root canal treatment and apicoectomy at the same sitting.  Generally I preferred doing both procedures      simultaneously because it was much easier to clean out the root canal that way.  In addition, we could spray a disinfectant through the root canal and vacuum the debris and infected material out from the root-end surgical area.</p>
<p>It was also easier to get a good dense root canal filling as any overpacking could easily be removed.</p>
<p>In my practice I did a fair number of these apicoectomy surgeries and I cannot recall any which did not show full healing at the end of the root of the tooth, usually within the period of one year. We were not universally successful with teeth treated nonsurgically.</p>
<p>A couple of case history reports from my files will allow you to see how well infections responded to the apicoectomy procedure.</p>
<p>The first is of a woman who came home from a trip to Mexico with severe diarrhea and intestinal involvement. When a leading Beverly Hills gastroenterologist  was unable to cure her intestinal infection, the patient, knowing I was doing nutritional counseling, sought my opinion.</p>
<p>Oral examination didn&#8217;t indicate any obvious pathologic conditions or tooth decay, but there were many fillings present.  Full mouth x-ray examination disclosed large abscesses from accessory canals on both of her upper lateral incisors (the teeth next to the two front teeth).</p>

<p><strong>Dental Hygiene – Is It A Good Career Option</strong></p>
<p>In order to practice dental hygiene, one must pursue a recognized dental health program and has to be registered with the provincial or territorial regularity authority. The dental hygiene program aims at providing the students with required knowledge and skills in the subject. The study material includes assessment of a patient’s general and oral health, removal of plaque, dental radiography, administration of local anesthesia, stain and calculus (tartar), oral hygiene instructions, dietary counseling, and applications of preventive agents (fluoride).</p>
<p>On completion of the program, the graduates have to take a national written and regional clinical examination. The successful candidates are then rewarded with the license to practice. There are numerous career opportunities for the successful graduates including clinical positions in dental offices, hospitals, public health agencies, community dental clinics, research and marketing of dental products, and educational and administrative positions.</p>
<p>The practice of dental hygiene is unstable currently in United States of America as the national organization of hygienists is lobbying for the right to administer their own independent practices – independent of direct supervision of dentists. Presently, independent hygiene practice in U.S. is restricted to the state of Colorado. While, there is an acute shortage of skilled dental hygienists in the country.</p>
<p>The registered dental hygienists (RDHs) may opt for a career in community as well as clinical practice. The dental hygienists often work under the supervision of the general practitioner dentists or specialty practice dentists. There, they are trained to work independently and directly with the patients to treat the gum diseases and tooth decay. The training includes assessing tooth and gum health, preparing individual dental hygiene treatment plans, and referring the patients to a dentist for dental treatment. Also, the dental hygienists are prepared to look after the cosmetic appearance of teeth and gums and to perform complex practices like scaling and root planning to treat gum infections and other related diseases. The states like Colorado allow dental hygienists to run their own dental hygiene practice. The dental hygienists here are free to practice independently without the supervision of a practicing dentist.</p>
<p>The dental hygienists can also work in community dealing with the entire community on one to one basis. A hygiene dentist may have to work in coordination with different health professionals and community members. They may have to perform health surveys for different communities and work with other health professionals to plan a health program for the community. Health professionals include nurses, injury prevention coordinators, speech pathologists, dentists, physicians, and dieticians. Whereas, the community members can be school principals, teachers, coaches, parents, health advocates, day care providers, government departments, administrations of other health promotion programs in the community.</p>
<p>So, the dental hygienists have a vast scope of career opportunities in different areas of health departments and programs. Choosing the dental hygiene as a profession leads to a respectable position with good amount of money as a reward. Presently, the dental hygienists have to work under a practicing dentist in the most parts of the country leading the dental hygiene professionals to work under the dentist. However, the efforts are on to promote the dental hygiene professionals opting to open their own dental clinic and work as an independent entrepreneur.</p>

The course is 30 hours covering all knowledge requirements detailed within the curriculum in Dental Radiography as set out by the British Society of Maxillofacial Dental Radiologists and certificated by the National Exam Board for Dental Nurses.</p>

			<content:encoded><![CDATA[<p><strong class="titler">X-Ray Pictures Fantastic but Limitation to Find Gum Infection, Root Canal infection</strong><br />

<p>Cheekbones, narrowness of jaws, the position of teeth and the formation of the hard palate make the taking of accurate x-ray pictures a challenge.</p>
<p>For these reasons x-ray pictures of teeth and jaws quite often fail to disclose the presence of infection or what kind of treatment might be required, contrary to generally accepted views of most dentists and patients.</p>
<p>Cracks in teeth are almost never visible on x-ray pictures.</p>
<p>The extent of bone loss around a tooth, though felt to indicate the severity of its infection, does not do so.  Doctors Basker and Stern, in separate studies, reported that 85 to 100 percent of dental granulomas and redicular cysts do not contain bacteria. This does not mean the tooth itself is not infected.</p>
<p>When the dentist removes or treats a tooth but the patients continues to have a toothache, it may seem the wrong tooth was removed. The pictures in this chapter show how a second tooth is often involved.</p>
<p>Because lateral canals can contain infection, dentists have been taught to advise root filled teeth be x-rayed each year or two.</p>
<p>None of this implies the use of x-ray pictures is to no avail. Quite the contrary, good x-ray pictures often disclose disease conditions which should receive immediate attention if the patient&#8217;s health is not to be jeopardized.</p>
<p>Both pointed out that the visual areas of abnormalities on x-ray pictures are not necessarily areas of infection harboring bacteria. Dr. Stern stated that, for the most part, such visible areas are lisions that have been identified as radicular cysts and granulomas.</p>
<p>The granulomas are reactive lesions caused by bacterial destruction originating from an infeced root canal that opens into that area. Experiments indicate the granuloma is a response of cells to the bacteria in the root canal, but, at the same time, bacteria cultures taken from the areas of lost bone at the root end of the tooth have proven negative 85 to 100 percent of the time.</p>
<p>These studies are yet another substantiation of discoveries made by Dr. Price in this regard some 45 years previous to the publication of the Baskar and Stern articles.</p>
<p>These are numerous other types of pathologic conditions which challenge the diagnostic ability of dentists. Some of these conditions are misinterpreted as tooth infections,but as descriptions of them would require      complicated explanations that are not germane to the purposes of this book, I mention them only to further indicate the number of difficulties inherent in the use of dental x-ray pictures in the interpretation of dental pathology.</p>

<p>What are dental implants?  How are Dental Implants placed? Read this article to understand the treatment used to replace missing teeth.</p>

<p>Dental implants are a wonderful way to replace one or more missing teeth.</p>
<p>Dental implants are very involved and time consuming, however; it is like having a natural tooth to replace the missing one.   Our patients are given the option of dental implants or a bridge to replace one or more missing teeth.</p>
<p>If a patient has bone loss throughout the mouth and has a lower denture that is loose, we recommend dental implants which can help keep the denture in place.</p>
<p>We like to inform our patients of all of their options and let them make an informed decision.  We do all procedures in our office without the need to refer to a specialist.</p>
<p>Does an insurance company cover dental implants? About half of the insurance companies are paying for dental implants.  We have discovered if they say a dental implant and related services are not a benefit, they consider paying as a bridge instead.  We offer CareCredit in our office to help our patients get their dental treatment with monthly payments.</p>
<p>In order for us to make an accurate diagnosis, we must have a current x-ray of the area.  If it is determined that our patient is a good candidate, we have them watch a video, we go over the consent form, and make financial arrangements.</p>
<p>The steps for a dental implant:</p>
<p>&#8211;Surgery:  the doctor makes an incision in the gum tissue, to place the implant post directly into the bone.  With the current x-rays that we have, he is able to determine which implant post is the correct one. He uses a drill to make an entrance site into the bone to place the implant.  An x-ray is taken to ensure that the implant is placed correctly.  Surgery is less traumatic than the surgery to remove the tooth.  Dr. Joe will place sutures that need to be removed by him.</p>
<p>&#8211;A post op appointment and a suture removal appointment are made.</p>
<p>&#8211;Usually three months after the initial surgery, we schedule an appointment for another x-ray to determine if the bone is healing around the implant.</p>
<p>&#8211;If the patient is healing well we will schedule them to uncover the implant post and attach an abutment and take an impression soon thereafter for a crown to be placed on the implant post.</p>
<p>&#8211;The impression is sent to the lab for a custom made implant crown.</p>
<p>&#8211;Three weeks later, your missing tooth is replaced with the crown and your treatment is complete.</p>
<p>So what does all of that mean?  The implant post is placed in the bone and the gum tissue and bone heal around it, (it is essentially the root of the tooth).  The abutment is the attachment that is placed on top of the implant post.  The crown is made and cemented to the abutment.  Your missing tooth is now replaced by a natural feeling and a natural looking dental implant and crown.</p>

<strong>Impacted Wisdom Teeth</strong></p>

<p>Wisdom teeth are the third pair of molars that appear during the late teens or thereafter. These are 4 in number. In some people, they may not appear at all.</p>
<p>An Impacted Wisdom Teeth are those wisdom teeth which do not emerge completely at their expected positions, due to a variety of reasons. This may happen when there is not enough space in the jaw to accommodate these teeth, or because the angle of the tooth&#8217;s emergence is not correct.</p>
<p><strong>Types of Impacted Wisdom Teeth</strong></p>
<p>When the wisdom tooth is angled forward, towards the front of the mouth, it called as a Mesial Impaction of the Wisdom tooth.</p>
<p>When the wisdom tooth is angled vertically, it is called a Vertical Impaction of the Wisdom tooth. When this happens, the tooth sinks into the jaw bone.</p>
<p>When the wisdom tooth is angled horizontally, it is called a Horizontal Impaction of the Wisdom tooth. When this happens, the wisdom tooth grows below the gums, and moves towards the roots of the neighboring tooth.</p>
<p>When the wisdom tooth is angled so that it goes back into the jaw, it is called a Distal Impaction of the Wisdom tooth.</p>
<p><strong>Reasons for Removal of Impacted Wisdom Teeth</strong></p>
<p>When you have Impacted Wisdom teeth symptoms of infection often appear because of food particles being stuck into the flaps of the gum. There may also be pressure on the neighboring tooth which may cause pain. Gum disease and its relevant complications are often the main cause for the removal of Impacted wisdom teeth.</p>
<p>The type of impaction can be ascertained by getting an x-ray of the area.</p>
<p>Often the jaw may be able to contain the wisdom teeth and they may have emerged properly, but these wisdom teeth are susceptible to decay and cavities. This is because the wisdom teeth are at the absolute end of the jaw. It may not be easy or possible to clean that area of the jaw properly, all the time. This may put the nearby teeth to a great risk. In this case, a dentist may also suggest a surgical removal procedure.</p>
